Covid 19 Program Registration
Register below for the TransFX COVID-19 program
To REGISTER
Have your insurance and vaccination cards ready.

- First you must establish your patient account with us. Do this by entering your email, date of birth and pertinent information the same way you would with any doctors office. Our system will confirm you mobile phone number via text message.
- Enter your vaccination status and upload a copy of your vaccination and health insurance cards as applicable. If you do NOT have health insurance or are NOT vaccinated, simply answer the provided questions.
- Once you have entered your information you will be asked to answer 4 short medical necessity questions. You will have to do this each week between Tuesday and Saturday. This should only take you less than 1 minute each time. This MUST be done weekly while you are registered in the program. These will document your exposure risks to the COVID-19 virus.
You will only have to register once, however you will be required each week between Tuesday and Saturday to log into your patient portal at https://www.qrmds.com/patient-log-in/ and answer the 4 short medical necessity questions. This should take you less than 1 minute each time. This is a $0 cost program to you at all times under the CARES Act. At no time will you be responsible for any insurance costs for patient co-pays or deductibles.
You will be asked to allow 3rd party cookies for our HIPAA data system to properly authenticate you. We do NOT in any way sell your information or data and are bound by privacy to keep your information protected under HIPAA. If you have issues registering on our site and/or have questions, you can email “support@qrmds.com” and we will respond usually within 24 hours or less. We have live 24/7/365 chat support via our website as well that can answer any non medical related questions.
The Testing Process
Step 1
Register Online
Enter your personal health information into our secure online database. Declare your vaccination status, as well as upload a copy of your vaccination and insurance cards.
Step 2
Medical Necessity
Each week, you will be asked to answer 4 short medical necessity questions which should take you less than one minute to answer. Do this by logging into your patient portal at https://www.qrmds.com/patient-log-in/ on your device and selecting “medical neccessity” from the menu.
Step 3
Take the test
If you are to test that week, you are assigned a vial code, and you will self collect your sample and then have a brief televisit interaction with one of our medical providers. This is done during the scheduled testing windows set up by your adminstrators at work.
Step 4
Get Results
Laboratory results are delivered to you on your mobile device via text message. Your results will also available in your patient portal. Lab results are delivered typically within two days. If you test positive, a medical provider will reach out to you directly.
