QRMDs Program for COVID-19
QRMDs medical services are provided by Dr. Steven Powell M.D. and we are licensed in every US state. Dr. Powell provides a $0 out of pocket cost COVID-19 testing. This program is based on insurance reimbursements via private insurance or the HRSA fund established by the CARES Act. All testing is performed only based upon documented ‘medical necessity’. If based on medical decision making guidelines as set forth by the American Medical Association, you do NOT have a reasonable exposure risk such that testing is supported, we will not test you under the program. Accordingly, each time before you test you MUST fill our a medical necessity verification form and your answers will determine your eligibility for testing based on medical decision making at the time. Any participant in a QRMDs group COVID-19 testing program will not be responsible for any co-pays, out of pocket expenses, or testing costs per the CARES Act and the Families First Coronavirus Response Act; (“FFCRA”). All medical services for QRMDs are provided by Dr. Steven Powell M.D. and staff.

*We require that you accurately state if you have private insurance. We are out of network for all insurance companies but accept all forms of medical insurance plans. You must qualify as being medically necessary to take a test.
ONLY medically patients that demonstrate exposure risk per current AMA guidelines, qualify for testing with QRMDs. We will not test any patient for COVID 19 that does medically qualify for testing.
Commercial Insurance Network
All medical services for QRMDs are provided by Medical Director, Dr. Steven Powell, M.D., and licensed, insured and qualified medical professionals under Dr. Powell’s direct supervision.
We are credentialed with most health care insurance companies, however we are NOT in network with any health plan or insurance network. Negotiated rate agreements can be discussed. Please contact Support@qrmds.com if you wish to negotiate a contract rate agreement.
Commercial Insurance Obligations to Pay Bills
The CARES Act requires commercial insurance carriers to pay all charges presented to them:
Section 3201 of the CARES Act amended section 6001 of the FFCRA to include a broader range of diagnostic items and services that plans and issuers must cover without any cost-sharing requirements, prior authorization, or other medical management requirements. Section 3202(a) of the CARES Act generally requires plans and issuers providing coverage for these items and services to reimburse any provider of COVID-19 diagnostic testing an amount that equals the negotiated rate or, if the plan or issuer does not have a negotiated rate with the provider, the cash price for such service that is listed by the provider on a public website. ( Families First Coronavirus Response Act (“FFCRA”) guidelines.
Negotiated Rates and Commercial Insurance Plans
We are happy to enter into a formal ‘Negotiated Rate Agreement’ (“NRA”) for costs and services with any commercial insurance plan. company plan or private entity for our services rendered. If you are a commercial insurance payer or private entity and would like to enter into a Negotiated Rate Agreement with us, you may begin that dialog by clicking this link here to begin that discussion. However, ANY AND ALL NEGOTIATED RATE AGREEMENT PLANS MUST PROVIDE FOR THE PROMPT AND FULL PAYMENT OF ALL CLAIMS UPON SUBMISSION IN A TIMELY MANNER AT THE FULL NEGOTIATED RATE. If you do NOT have a Negotiated Rate Agreement, then you must pay our list price for services pertaining to COVID-19 care and testing at the prevailing rates set forth below on our ‘cash payment rate sheet’.
Cash Price for Services
Pursuant to the FFRCA, we are required to post Dr. Powell’s non negotiated cash price for services (“Cash Price”) for all services rendered by Dr. Powell and his staff. Note that our Cash Price can change without notice. All changes to our cash price will be posted on our website.
This is the current price list updated as of March 2nd, 2022.
*At no time will QRMDs collect and process a test billed to insurance companies without documenting medical necessity and sufficient basis for exposure risk, per AMA guidelines, to authorize a test.
All pricing is subject to change without notice and is due upon service being provided. However, in no case beyond net 10 days. Unless subject to a negotiated rate agreement.
COVID-19 Testing:
RT-PCR Testing: CPT Codes: U0004/U0005 individually processed or sequenced, RT-PCR, test not pooled. FDA EUA approved: $375.00
Rapid Antigen COVID-19 Testing: CPT Codes: 87426 FDA EUA approved, point of care CLIA waived tests, individually administered (requires physician services and medical necessity verification): $125.00
G2023 Collection Code and fee for sample collection if physician televisit is not able to be performed. Note, all COVID-19 tests require physician oversight or physician order for the test, as well as medical necessity verification: $50.00

General Guidelines and Group Testing for Individuals and Costs.
There are no direct costs born by any participant in any group testing program. Further, no participant will be required to pay any co-pay charges, deductible expenses as per the requirements of CARES Act and the FFCRA. We are able to provide a no direct cost program pursuant to the FFCRA providing the ‘medical necessity’ is documented for each patient encounter and test. All testing requires physician supervision and oversight as well as a physician order for the test to be performed. We do NOT accept physician orders from any medical professional other than those contracted directly by SOS Environmental Health LLC/QRMDs, to perform those services. We are happy to share your medical records with your primary care physician at any time. All such requests must be in writing.
