How to ADD your child to your account

Registering Your Minor Child and Adding an Additional Patient to Your Account

Please follow these steps to register your minor child and add a patient to your account.  Remember that you MUST first register yourself and then ADD your minor children to the account one by one. 

Step 1
AFTER you have created your account

After you have created your account, log in to your patient portal by clicking on this link: https://www.qrmds.com/patient-log-in/ From the menu, select “Add Additional Patient” (see image).   If you are having problems logging into your account click here

Step 2
Add your minor child

When you see this screen, enter the appropriate information for your minor child.  You have to add your minor children one at a time.

Step 3
Make sure you associate your minor child with the correct testing program

When you registered, the system asked you if you were a part of a testing program.  Hopefully you set YOUR registration as part of your individual testing program for School or Institution.  You can check this by logging in to your patient portal https://www.qrmds.com/patient-log-in/ and viewing which program YOU are associated with.  IF you are associated with the correct testing program, the system will automatically associate YOUR minor child (additional patient) to the same program.  Under the question “is the patient listed above taking part in a group program?” Make sure you answer YES.  Select YES to the next question “is the test program the same as the other name(s) registered on your account?”   

You can check to see if you did this correctly by going reviewing your registration information. To do so log in to your patient portal and click on the “Patient Profile” link and verify the information.  Be sure your account is associated with the the correct testing program.  

Step 4
Entering your minor child's insurance information

Please complete this section and upload a digital copy of the insurance information.

Step 5
The "Medical Necessity Verification"

After you have entered your minor child’s information, you will be asked to answer 4 brief “Medical Necessity Verification” questions. This should take you only seconds.  

YOU MUST ANSWER THESE SAME QUESTIONS EACH WEEK PRIOR TO TESTING YOUR CHILD, AT LEAST ONE DAY BEFORE THE TEST DAY.  

You will answer these questions initially when you register your minor child.   You can answer the questions as early as one week before a test day, but no LATER than 1 day before the test itself.  If you do NOT answer the questions, when your child shows up at school to take the test, their name will show on the school list as “needs medical necessity verification” and they will NOT be able to take the test.  

Remember to each week answer the medical necessity verification questions on behalf of your child.

After this you should be registered
